This form allows players to accept an invitation to attend a South-run Performance Centres.

Once submitted this form is not updatable. If you have made an error or there are changes please submit a second one. When the forms are processed the final one will be the one that is used.

If the submission is successful you receive a thank you message on the screen. There is no automatic email to confirm the entry.

This form is best completed using Windows. It has been known to encounter errors which prevent the form from being submitted using other environments

We will be requiring umpires for the selection day. Please mark any parent who will be accompanying the child who is able to assist.

For those at boarding school we are able to allocate the places based on either their home or school address. It may be possible to use an alternative address, but please complete this form and contact Tony separately.

There are ten sessions due on Monday evenings in the next cycle. Consequently, we need to know the following details for the school which the player is likely to attending from September.

First half of postcode is all that is needed, the town if postcode is not known.
